Abstract : We present a case of neglected atypical high grade prostatic stromal sarcoma (PSS) at the transitional zone, invading the peripheral zone, prostatic capsule & peri prostatic fat reaching to pelvic side wall, it showed T2WI hyperintense signal (compared to typical cases of PSS showing T2WI hypointense signal) corresponding to developing on top of pre-existing nodular hyperplasia. Careful reviewing of prostatic central / transitional zone lesions is mandatory especially at T2WI, diffusion & contrast images to exclude malignancy which can be easily missed due to multiple signal patterns described. Most common tumors at transitional zone is stromal sarcoma which could be typical showing T2 hypointense signal or atypical showing T2 hyperintense signal intensity.

A Histological and Radiographical Comparison Between Split-bone block and Cortico-Cancellous Block Graft Utilized for Horizontal Ridge Augmentation

Abstract : This randomized clinical trial evaluated the histological and radiographical characteristics of bone regeneration after horizontal alveolar ridge augmentation using two different surgical approaches. The study randomized 20 patients into two groups: split-bone block group and cortico-cancellous bone block group. The grafts were obtained from the mandibular symphysis in both groups. Radiographic bone density of the augmented sites was measured by CBCT scans at baseline and 6 months post-operatively. A bone biopsy was taken at 6 months post-operatively before implant placement. Histological evaluation showed significant differences in bone tissue percentage between the groups. The split-bone block group had a higher mean percentage (61.80%) than the cortico-cancellous block graft group (46.90%). Radiographic bone density at 6 months post-operatively also differed significantly between the groups, with the split-bone block group showing a higher mean density (P = 0.000). Within the limitations of our study, we found that the split-bone block graft yielded superior bone regeneration quality than the cortico-cancellous block graft.

A Comparison Between Split-Block Graft and Cortico-cancellous Block Graft for Horizontal Ridge Augmentation: A Randomized Clinical Trail

Abstract : The alveolar ridge undergoes both horizontal and vertical resorption following tooth extraction, accompanied by alterations in the structure and composition of the overlying soft tissues. These changes compromise the optimal three-dimensional implant placement and necessitate bone augmentation procedures in such scenarios. The aim of this study was to compare the dimensional changes after horizontal alveolar ridge augmentation for the anterior maxilla, using split-block bone graft and cortico-cancellous bone block. This randomized controlled trial compared two types of bone blocks for alveolar ridge augmentation: split-bone blocks and cortico-cancellous bone blocks. Both types of grafts were harvested from the mandibular symphysis of 20 patients, who were randomly allocated to either group. Cone-beam computed tomography (CBCT) was used to measure the horizontal ridge width before and 6 months after the surgery. Before augmentation, there was no significant difference in the mean alveolar ridge width between the two groups (P = 0.415). However, after the surgery and at 6 months post-operatively, the mean alveolar ridge width was significantly higher in the split-bone block group than in the cortico-cancellous block group (P = 0.035 and P = 0.013, respectively). Within the limitations of our study, we found that the split-block bone graft is more effective than the cortico-cancellous bone block for horizontal ridge augmentation, as it achieved higher ridge width after 6 months of follow-up.

Abstract : There are more elderly people being admitted to intensive care units (ICUs) due to an increase in the prevalence of severe sepsis and septic shock. Sepsis is more likely to affect the elderly due to co-morbid conditions, frequent and lengthy hospital stays, lowered immunity, functional limitations, and, most importantly, the effects of ageing itself. This systematic review aims to study the recent updates regarding the outcomes of elderly patients admitted to the ICU for sepsis. PubMed, SCOPUS, Web of Science, and Science Direct were systematically searched for relevant literature. Rayyan QRCI was employed throughout this comprehensive process. Our results included eight studies with a total of 29754 patients, and 15681 (52.7%) were males. In ICU, patients with sepsis, older age and elderly patients were risk factors for mortality. Fever and significant function loss were these individuals' most prevalent symptoms. High levels of HCT-ALB are linked to an increased risk of hospital and intensive care unit deaths. There is a dearth of knowledge on severe sepsis in the very elderly, especially regarding variables influencing prognosis, quality of life, and functional success following sepsis therapy. Since it is often believed that they will not respond to treatment as well, the very elderly are typically excluded from antisepsis and antimicrobial agent trials. To create optimal healthcare plans as the population ages and sepsis cases increase, future studies should, however, also focus on this patient age group. The effectiveness of prophylactic measures and the use of bundled approaches in the care of elderly patients with severe sepsis should be evaluated in further research.

Promotion of Dental and Oral Health Through The Gigiku Sehat Application and Healthy Dental Gymnastics With Teacher Assistance on Teeth Cleaning Behavior in Children Aged 10-13 Years

Abstract : Caries is one of the oral diseases that is often experienced by children around the world by 60%-90%, especially in low socioeconomic groups. The causes of caries are multifactorial, including socioeconomic level, age, gender, diet and oral hygiene. Sucrose or sugar commonly consumed is an important factor causing caries. Elementary school children are an age group that is very vulnerable to caries because they have poor behavior in maintaining oral and dental health. Aim: Knowing the effect of healthy teeth application and healthy dental gymnastics with teacher assistance on teeth cleaning behavior in children aged 10-13 years. Method: The study was conducted in September 2022 - March 2023 at Makassar City Elementary School, South Sulawesi. The number of samples were students of SD Hang Tuah 64 people, SDN Ujung Tanah 54 people and SDN Cambaya 49 people. The research group consisted of 3 (three), namely SD Hang Tuah with a combination intervention of healthy dental application and healthy dental gymnastics, SDN Ujung Tanah with healthy tooth application, SDN Cambaya healthy dental gymnastics. The three research groups intervened for 3 (three) with teacher assistance. There are two stages in this study, namely stage 1 of making healthy dental applications with qualitative methods followed by stage 2, namely educational model intervention with quasi-experimental methods with the design of the non randomized pretest postest control group design. Results: In the knowledge aspect, there was a significant increase in SD Hang Tuah (p = 0.021), SDN Ujung Tanah (p = 0.000) and SDN Cambaya (p = 0.015). In the attitude aspect, there was a significant increase in SD Hang Tuah (p = 0.039), SDN Ujung Tanah (p = 0.001) and SDN Cambaya (p = 0.000). In the Action aspect, there was a significant increase in SD Cambaya (p = 0.043) while SDN Hang Tuah (p = 0.177) and SDN Ujung Tanah (p = 0.249) experienced a significant increase but there was an increase in the mean value. Conclusion: The effect of healthy teeth application and healthy dental gymnastics with teacher assistance has an effect on increasing knowledge, attitudes and actions in maintaining oral health of children aged 10-13 years.
