Abstract : Saudi Arabia has integrated preventive and primary curative services through primary healthcare centers (PHCCs) but lacks adequate mental health services. To address this gap, the Ministry of Health established the Primary Mental Health Care (PMHC) program in 2016, aiming to improve early detection, diagnosis, and management of mental disorders at PHCCs. This study aims to describe the demographic and clinical characteristics of patients referred to the mental health unit in Buraidah city, Qassim province, in 2020-2023. This retrospective cross-sectional study analyzes the medical records of patients referred to the mental health unit in Buraidah city, Qassim province, during 2020-2023. The study population includes all patients referred to the unit in 2020-2023, with inclusion criteria requiring complete medical records. Data, collected via a Google Forms questionnaire, comprises patient demographics, mental illness history, past medical history, provisional diagnosis, PHQ-9 and GAD-7 interpretation, patient response to management, and management methods. This study analyzed records of 200 patients from a mental health unit. Generalized Anxiety Disorder (GAD) and Major Depressive Disorder (MDD) were the most common diagnoses, affecting 38.9% and 38.0% of patients, respectively. The majority of patients reported an impact on sleep, performance, and relationships due to their mental health condition. Most patients were managed with medication (60.0%), while others received Cognitive Behavioral Therapy (26.8%) or social support (13.2%). The level of depression was significantly associated with the number of visits, marital status, and occupation. Marital status was significantly associated with anxiety severity, while other demographic factors showed no significant association. This study showed a higher prevalence of mental disorders among females and young adults. Generalized Anxiety Disorder and Major Depressive Disorder were the most common diagnoses. Mental health conditions significantly impacted patients' lives, particularly sleep, performance, and relationships. Pharmacotherapy was the most common treatment, followed by psychotherapy. Demographic factors were significantly associated with depression and anxiety severity, emphasizing the need for targeted prevention and intervention strategies for at-risk populations.

Restoration of Grossly Decayed Primary Upper Incisors using Dentine Posts under Oral Sedation
Journal ID : AMJ-26-06-2023-11549; Author : Ayham Hijaz, Mohamed Altinawi, Imad Katbeh, Anas Abdo, Hasan Alzoubi,
Abstract : Severely damaged primary anterior teeth that require pulp therapy present a high risk of failure due to the loss of tooth substance, resulting from pre-existing decay and endodontic therapy. The ideal post material should have physical and mechanical properties similar to those of dentin. Another concern in restoring endodontically treated primary teeth is the need to find a material that can resorb similar to the natural tooth structure as a part of the exfoliation process, allowing normal eruption of permanent successors. Accordingly, there is no such material other than dentin itself. The introduction of biological dentin posts offers an excellent alternative for restoring such teeth. This report presents restoring four primary incisors using biological dentin posts for 12 months follow-up.

Comparing the clinical performance of high viscosity bulk-fill composite with conventional composite in children aged 5-8 years: A Randomized Controlled Trial
Journal ID : AMJ-18-06-2023-11545; Author : Maram Alsaytari, Mohamed Altinawi, Hasan Alzoubi,
Abstract : This randomized controlled clinical trial is aimed to evaluate the clinical performance of high-viscosity bulk-fill composite in the restoration of second mandibular primary molars compared to conventional incremental composite. The studied sample consisted of 60 second mandibular primary molars in 59 children aged between 5-8 years, these cases were allocated randomly into four groups: Group 1 (experimental, n=20): High Viscosity Bulk Fill Composite with total etching (HVBFC-T), Group 2 (experimental, n=20): High Viscosity Bulk Fill Composite with Self-etching (HVBFC-S), Group 3 (Control n=20): Conventional Incremental Composite with Total etching (CIC-T). Post-treatment clinical follow-ups were conducted during 3, 6, and 12 months using The Modified United States Public Health Service (USPHS) criteria. Data were analyzed using Kruskal- Wallis test. The clinical success rate reached 100%, 95%, and 95% in HVBFC-T, HVBFC-S, and CIC-T groups respectively. Statistical analysis revealed no statistically significant differences among HVBFC-T, HVBFC-S, and CIC-T groups in overall clinical evaluation (P-value= 0.375). Bulk fill composite with two dentine bonding systems (Total Etch, Self-Etch) showed satisfactory clinical performance in primary molars during the 12-month follow-up period.

Potention Black Soldier Fly's (Hermetia illucens) Live for Wound Healing and Bone Remodeling: A Systematic Review
Journal ID : AMJ-15-06-2023-11542; Author : Renie Kumala Dewi, Sri Oktawati, Asdar Gani, Eko Suhartono, Nurlinda Hamrun, Lailatul Qomariyah,
Abstract : The Black Soldier Fly (BSF) (Hermetia illucens) is included in the decomposer fly which is not a disease vector. BSF is widely cultivated because it has high nutritional value with a protein content of 44.26% and a fat content of 29.65%. BSF life has the potential as a raw material in the chitosan manufacturing process because it contains the chemical compounds chitin and chitosan. Chitosan is able to accumulate collagen fibers and produce faster wound healing and as an alternative material for bone remodeling. To find out the potential of BSF life as wound healing and bone remodeling. Database obtained from PubMed, Science direct, Google Schooler databases for the last 5 years from 2019-2023. The keyword used in this research is BSF. The study selection process was included in the data meta-analysis using PRISMA-2020. Screening of full text articles was included in a systematic review based on inclusion and exclusion criteria. The search resulted in a total of 18 articles being entered based on the eligibility criteria. The study search categories obtained include BSF characteristics including larvae (maggot), pupae and shells as wound healing and bone remodeling materials, information related to chitosan from BSF, namely the method of making chitosan, characteristics of chitosan test results, the benefits of chitosan in the process of wound healing and bone remodeling. The purity of BSF chitosan is proven through the FTIR, XRD and TGA functional groups which are recognized by FDA (Florida Dental Association). Chitosan is a bioactive material in bone tissue engineering which can be processed in the form of a gel or scaffold. Chitosan improves osteogenic performance so that it can be used as a material for the treatment of bone defects by increasing the expression of Osteporotegerin (OPG), osteocalcin, BMP-2, osteoblasts and decreasing proinflammatory cytokines IL-6, TNF-α, RANKL which have all achieved good therapeutic effects.
