Magnesium level among patients with sickle cell anemia: A meta-analysis

Abstract : Low levels of serum magnesium have been associated with increased polymerization and sickling and consequently vaso-occlusion (VOC) in sickle cell anemia (SCA) patients due to cell dehydration. The K-Cl co-transport plays a very important role in sickle cell dehydration and is inhibited by significantly increasing levels of magnesium. In this meta-analysis, we evaluated total serum magnesium levels in SCD patients and “healthy” controls. We pooled six studies (all from Africa and two were published in Asia).626 patients were included. The keyword used were, sickle cell disease, sickle cell anemia, and serum magnesium levels. The dichotomas data were entered manually in the most recent RevMan system(version,5.4) for metanalysis. The random effect was chosen due to the significant of heterogeneity. Funnel plot was used to assess lateralization. A P-value of <0.05 was considered significant. We found a lower serum magnesium level among patients with sickle cell disease compared to their counterparts controls, odd ratio,-.024.95%Cl,-.058--.011, Chi-square,184.07, and P-value for overall effect,0.005. A substantial heterogeneity was found, I2 for heterogeneity,98%, P-value for heterogeneity <0.001. The study shows that there is lower serum magnesium level among patients with sickle cell disease as compared to their controls. Magnesium homeostasis are altered in SCD patients, and their levels are lower in HbSS patients. Magnesium supplementation may be required to be supplemented to sickle cell anemia patients.

Stress Urinary Incontinence (SUI) Risk Factor in Pelvic Floor Muscle Strength (PFMS) among Postmenopausal Women: A Case-Control Study

Abstract : Postmenopausal health problems cause many symptoms that affect the quality of life among women, including the reduction of pelvic floor muscle strength (PFMS). Stress urinary incontinence (SUI) is one of the postmenopausal women’s problems due to various etiology. Risk factors analysis and stratification are important for further prevention and treatment. A case-control study that collects postmenopausal women divided into two groups, 52 postmenopausal subjects with SUI and 52 without SUI. Pelvic floor muscle strength is measured by a perineometer (Pertiron 9300 V). There is a correlation between the occurrence of SUI with parity (OR=11.111 95% CI 2.403-51.371) and the delivery mode (OR=8.704 95% CI 2.984-25.387). The cut-off value of PFMS to predict SUI is 27.03 cmH2O with 84.6% of sensitivity and 84.6% specificity. Our study reveals PFMS in combination with parity and delivery was simultaneously significant to the occurrence of SUI. Pelvic Floor Muscle Strength among menopausal women is correlated with SUI. Risk factors of SUI related to the reduction of PFMS are multiparity and vaginal delivery.

Analysis of Factors Influencing The Participation of Elderly Posyandu at Tulehu Health Center

Abstract : The Integrated Service Post (Posyandu) is a meeting point between professional services from health workers and community participation in tackling public health problems, especially in efforts to maintain good health status for the elderly. The role of cadres in organizing the elderly Posyandu is as an activity planner, communicator, mobilizer and service provider. This study aims to analyze the factors that influence the participation of the Elderly Posyandu at the Tulehu Health Center. This type of research is a qualitative research with a cross sectional study approach. Respondents in this study were obtained using the purposive sampling method as many as 227 elderly using a questionnaire in the form of a questionnaire. Data processing was carried out using SPSS and for data analysis using statistical tests using descriptive statistical tests, comparative analysis tests and multivariate analysis tests. The results showed that the knowledge factor, the need for health services in the elderly had a significant effect on the participation of the elderly in the elderly Posyandu in the working area of the Tulehu Health Center. The variable need for health services in the elderly is the variable that has the strongest relationship compared to other variables. Meanwhile, the family support variable has no relationship with the participation of the elderly in the elderly Posyandu. Suggestions from this study are for elderly posyandu cadres and officers at the health center, especially in the working area of the Tulehu Health Center, it is hoped that they can make supporting activities or improve elderly posyandu services so that they motivate the elderly to participate in elderly posyandu activities such as creating creative handicraft activities, providing additional food, doing arisan for the elderly, routine and complete health checks, as well as specifically for cadres to be more active and active in socializing and making the activities of the elderly posyandu successful.

Analysis Determinants of Stunting in Toddlers on 12 Locus of Ambon City

Abstract : Stunting is the biggest threat to human quality of life in the future because it can inhibit physical growth, inhibit children's brain growth (cognitive), reduce the quality of learning to decrease productivity in adulthood and increase the threat of non-communicable diseases. Stunting is caused by low nutritional intake and recurring illnesses which are reinforced by an unhealthy environment. This study aims to analyze the determinants of stunting in toddlers at 12 loci in Ambon City. This type of research is quantitative research with analytic observational methods, namely research that is directed to explain a situation or situation. Sampling was carried out through two stages, namely the purposive sampling method and the simple random sampling method with a total sample of 371 toddlers. Data processing is done using SPSS. The results showed that there was a significant relationship between history of breastfeeding, number of calories, parents' income and the condition of the drinking water source with the incidence of stunting, while the sanitation variable was not related to the incidence of stunting. Suggestions for the Puskesmas, namely that the Puskesmas is expected to conduct counseling and health education regarding stunting in the community, for the community, namely parents of toddlers, they must pay more attention to the number of calories/day in the form of macronutrients and micronutrients and in the first 6 months of life are obliged to provide exclusive breastfeeding, the condition of water sources drink so as to prevent stunting in toddlers. As for the advice for the Health Service, this index is expected to be considered and utilized as a measuring tool in assisting health workers.

The Effect of Human Resource Management on Performance of Employees at Health Training Center of Maluku Province

Abstract : Health Training Center is able to realize and increase the capacity of professional and competitive health human resource through training and development for quality and qualified health workers. Human Resource capacity building was carried out through human resource management process so that it improves the performance of Health Training Center employees. This study aimed to analyze the effect of planning, recruitment, compensation, employee relation on employee’s performance which conducted at Health Training Center of Maluku Province. The study design was qualitative descriptive. The sample in this study was 9 people which consisted of 3 key informants and 6 additional informants obtained using purposive sampling. Method of data collection in this study used interview method. The data that has been collected, then analyzed using the Miles and Huberman model. The results of this study indicate that Human Resource planning is carried out every year by analyzing employee needs, then submitted to the BKD of Maluku Province. BKD would conduct recruitment with qualifications based on the need. Compensation given to Health Training Center employees of Maluku Province is in accordance with a power of attorney issued by the leader. The s at Health Training Center of Maluku Province is very good, but there are still employees who are grouped, not harmonious, do not understand each other, communication is not really good. Based on the results above, it can be concluded that human resource management has an effect on the performance of employees of Health Training Center of Maluku Province in general.
