Precocious Puberty Presentation in a Child with Peutz–Jeghers Syndrome - Case Report

Abstract : Peutz–Jeghers syndrome (PJS) is a rare inheritable condition characterized by hamartomatous polyposis of the gastrointestinal tract, with oral mucocutaneous pigmentation. We report the case of a 6-year-old boy who presented with gynecomastia and testicular enlargement. The family history was interesting for different types of tumors from the maternal side. Moreover, the examination revealed incidental peri-oral and buccal lentigines spots. Most endocrine presentations in boys involve testicular tumors, precocious puberty, and tall stature. Regular clinical follow-up is essential to evaluate the intestinal polyps, endocrine function, and tumors in these patients.

Perception of medical students regarding academic advising in Tabuk City, Saudi Arabia.

Abstract : Academic advising is vital for career development and better student achievement. No researchers have evaluated the pattern of and the student's perception of academic perception. This study aimed to assess the same among Saudi medical students at the University of Tabuk. We conducted a cross-sectional among randomly selected 248 medical students, at the Faculty of Medicine, University of Tabuk, Saudi Arabia. The study was conducted during the period from March 2023 to April 2023. A structured questionnaire based on demographic data, the pattern of academic advising, and the student's perception of academic advising was used. Out of 248 medical students (58.1%) were from the basic years), more than one-third (36.5%) of the students thought that academic advising is not important, and 60.5% and 44.8% suggested that it should be optional and target-specific groups of students. The sessions took 10-15 minutes in 79%, the students preferred afternoon and evening sessions (82.7%), and one-to-one in 77.8%. The perception of medical students of academic advising is good, 73.4% believed that communication is good, and 68.5% were convinced about their adviser's help, preparation, and knowledge. The students thought that their adviser was familiar with their academic background, a good listener, and easy to approach 60.4%, 70.1%, and 81.1% respectively. The academic advising session was short, ranging from 1-7session/per year, and the students prefer afternoon and evening sessions. Although, the students received academic advising as positive, a considerable percentages need to be convinced. The students found that their advisers were knowledgeable, easy to approach, and good listeners.

ONE STAGE OSTEOTOMY AND TKA IN OSTEOARTHRITIS OF KNEE WITH EXTRA ARTICULAR DEFORMITY POST FREE VASCULARIZED FIBULAR GRAFT

Abstract : One of the main treatments for lateral knee osteoarthritis with valgus malalignment is tibial medial closing wedge osteotomy (CWO) followed with total knee arthroplasty (TKA). One stage osteotomy and TKA in osteoarthritis with extra-articular deformity is difficult yet shown to be effective. We report a case of CWO and TKA as a surgical treatment for a patient with osteoarthritis and malunion of the right middle shaft of the tibia following a Free Vascularized Fibular Graft (FVFG) procedure. The patient mainly complains of limping in the right lower leg. He had been suffered from this complaint for almost 20 years and worsen last decade. The patient also complained about crookedness in his right lower leg. The right knee revealed valgus deformity and shortening of the limb with leg length discrepancy 9 cm. The patient was planned for one-stage realignment with CWO and TKA. FVFG has a high complication rate such as mechanical failure, graft fracture, non-union, infection, and other associated complications at the donor site. Following this condition, although not common, CWO is used as a treatment choice for alignment surgery, especially for malunion, realignment and shortened leg. TKA then follows CWO for the correction of osteoarthritis in one stage operation CWO followed by TKA is management that would realign the anatomic position and maintain patient satisfaction for a longer period. These procedures provide high clinical efficacy in restoring the deformity that showed in our patient.

EVALUATION OF URINE CULTURE BEFORE AND AFTER CATHETER INSERTION IN ELECTIVE AND EMERGENCY CAESAREAN SECTION

Abstract : Indwelling urinary catheterization is a routine procedure before a cesarean section. The catheterization can cause bacterial colonization of the urine and cause urinary tract infections. Catheter-Associated Urinary Tract Infection (CAUTI) is the cause of 40% of all nosocomial infections worldwide. This study is an analytic study with a cross-sectional design at Khadijah Hospital and Fatimah Hospital in Makassar City. Urine culture examination was performed on 99 patients who were going to undergo elective and emergency cesarean sections before and after catheter insertion in cesarean section procedures. Data were analyzed using the Chi-square test, Mc Nemar, Mann-Whitney test, and t-independent test. The study showed that urinary bacterial colonies before catheterization were found in 18.60% of elective cesarean section patients compared to only 10.71% after catheter insertion. The urinary bacterial colonies before catheterization in 2.33% of emergency cesarean section patients were 14.29% compared to after the catheterization. There was a significant difference in the results of urinary bacterial colonies before and after catheterization in patients with elective cesarean section with a p-value was < 0.05. However, there was no significant difference in the results of bacterial colonies in the urine between pre and post-catheterization in patients with an emergency cesarean section and a value of p > 0.05. There was a change in urine culture before and after catheterization, more colony growth was found in an elective cesarean section than in an emergency cesarean section, and bacterial colony growth was also more common preoperatively than postoperatively.

Health Risk Analysis of Motor Vehicle PM2.5 Exposure Against Street Traders In The City of Makassar

Abstract : Air pollution is a very complicated problem because it involves matters relating to physical characteristics, sources of pollutant emissions. The purpose of the study was to analyze the level of health risk (Risk Quotient), due to PM2.5 particulate ambient air on Jalan A. P. Pettarani. Including, duration of exposure, frequency of exposure, and duration of PM2.5 exposure at the AP road congestion node. Pettarani. The target population in this study were street vendors along Jalan A. P. Pettarani, the number of samples was 48 traders. The results obtained, the maximum concentration of 2,458 – 3,409 mg/m3. The duration of exposure (tE) varied between 12.53 – 16.60 hours/day, the frequency of exposure (FE) ranged from 187.06 - 260 days/hour, and the duration of exposure (Dt) 11.57 years – 16.08 years. Inhalation rate 0.61 – 0.64 m3/hour, PM2.5 intake obtained between 0.061 – 0.972 mg/kg/day with a range of 5 – 30 years. Risk level (RQ) 3.389 – 53,979 with forced lung vital capacity 77.08% abnormal, 22.92% normal. The concentration of PM2.5 exceeds the threshold, which is very risky for traders, this is obtained from the examination of vital lung capacity which is more abnormal.
