RES-CMCS Modulates SMAD-2/SMAD-3/TGF-β/TGFβR signalling Pathway in DMH-induced colon carcinoma

Abstract : The bio-functional molecule RES is present in grapes and mulberries. The current study aims to green synthesis of RES-CMCS nanoemulsion with low viscosity chitosan to test its protection of colon against carcinoma. The RES-CMCS produced was characterized using Zeta potential, particle size, morphology, and entrapment efficacy measurements. It was also examined for its colon protective activity against DMH induced colon carcinoma in mice model. The RES-CMCS had spherical shape with a mean particle size of 54.7 nm, a zeta potential of (-) 59.4 mV, and an entrapment efficacy of 85.46 %. Exposure of mice with RES-CMCS and 5-Fluorouracil, individually and/or in combination significantly normalized colon CAT, GSH, NO, SOD, COX-2, TNF-α, caspase 3 and CEA as well as SMAD-2, SMAD-3, TGF-β and TGFβR genes expression when compared to DMH administrated mice. The findings show that RES-CMCS could be employed as a novel pharmaceutical medicine strategy in the protection of colon against cancer by regulation of antioxidant enzymes as well as inflammatory mediator's gene expression.

Could Dexmedetomidine be the Game Changer? Investigating its Viability as an Alternative to Adrenaline in Local Anesthesia: A Randomized Controlled Split-Mouth Clinical Trial.

Abstract : The aims of this study are to highlight the risks associated with using topical drugs containing vasoconstrictors, especially in high-risk patients, and to find an alternative to adrenaline for local anesthesia in this experimental research. This randomized split-mouth controlled clinical study involved 80 patients undergoing orthodontic dental treatment, with 160 upper premolar extractions performed in a symmetrical manner. The average age of the patients was 19.75 years. Each patient's sample was divided into two symmetrical sides. The first side was anesthetized using lidocaine/adrenaline (the Control Group). The onset time of anesthesia, duration of action, pulse rate (PR), and peripheral blood oxygen saturation (SpO2) were recorded. The other side was anesthetized using lidocaine/dexmedetomidine (the experimental Group), and there was a five-day interval between the two interventions. Findings revealed no statistically significant difference in the onset time of anesthesia between the two groups (P=0.345). However, there was a statistically significant difference in the duration of anesthesia, with the adrenaline group showing a longer average duration compared to the dexmedetomidine group (P=0.001). There was no statistically significant difference in peripheral blood oxygen saturation (SpO2) in the two groups. Nevertheless, there was a statistically significant difference in pulse rate, with adrenaline causing an increase in pulse values (P=0.001), while dexmedetomidine caused a decrease in pulse (P=0.001). Dexmedetomidine may offer potential benefits in terms of improving the onset time of anesthesia, maintaining peripheral blood oxygen saturation (SpO2), and causing a reduction in pulse rate below normal levels compared to adrenaline as a local anesthetic alternative.

Short-term effectiveness of modified removable mandibular retractor appliance for treatment of class III malocclusion: A Clinical Randomized Controlled Trial

Abstract : This study aimed to investigate short-term outcomes of hard- and soft tissue changes induced by modified removable mandibular retractor appliance in Class III patients and compare them to those of conventional removable mandibular retractor appliance. The sample included 40 patients, their ages ranged between 7-9 years, who were randomly divided into two groups: Group1: Twenty patients treated with a modified mandibular removable retractor device(M-RMR). Group 2: Twenty patients treated with a conventional mandibular removable retractor device (RMR). All patients had skeletal class III with crossbite on at least two anterior teeth. Cephalometric images were performed for each patient before applying the device T1, and after obtaining a positive overjet (2-3) mm T2, the changes between (T1-T2) were studied for each group. Then the results were compared between the two groups. The results showed better sagittal progression of the maxillary in the M-RMR group compared to the traditional RMR. slight increase in the vertical dimension of the face in both devices, and a noticeable improvement of the soft tissues in both devices. The modified mandibular retractor device showed effective treatment of skeletal class III. The presence of the lip- pads was very effective in maxillary protrusion.

Lemuru Fish Nuggets with Durian Seed Flour (NUGILEMPUDU) to Overcome Stunting on 10-11 years Children

Abstract : Stunting, especially on school children, has an impact on intellectual quality which can be measured through learning achievement. The condition of stunting describes the quality of nutrition in the past due to deficiency of nutritional intake needed during linear growth, especially protein, zinc, magnesium, calcium and serum feriitin. In addition, it is also caused by environmental factors, one of which is poor sanitation that causes diseases related to digestion, causing the process of absorbing nutrients, especially inadequate micronutrients. This condition will certainly continue to inhibit IGF-1 receptor expression for longitudinal bone growth. Therefore, it is necessary to strive for a food development model that becomes an alternative for stunting prevention/overcoming, which can still be pursued in the growth of the prepuberty/puberty period with the age of 10-11 years. This research is a quasi-experiment that will be tested on stunted children with a pre-post test design. The intervention carried out in this study was the provision of lemuru fish nuggets durian seed meal (nugilempudu) to stunted children. Before the intervention, TB and several other assessments were measured such as blood albumin levels, zinc, magnesium, calcium and serum ferritin. TB measurement using TB / U indicator, the results will be seen using the Z score method. The difference between the Z score obtained before and after the intervention will be tested with the laboratory values mentioned earlier. The results of this study are expected to be an additional food that can improve the nutritional status of the community and prevent nutritional deficiencies on school children in particular.

Awareness and Perception About the Specialty of Family Medicine Among Saudi Arabia.

Abstract : In recent days, the family medicine specialty growing as a potential branch to meet the needs of the community as a basic-level or first-level contact physician. From this perspective, need for a lot of awareness among the general population to utilize quality family medicine services in a better way. The main objective of our study was to find the general population’s awareness of family medicine specialty and demographic factors associated with satisfaction. A cross-sectional study was conducted among the general population through google forms during the period from July 2022 to March 2023. Sample of 385, the calculation was done based on the Open Epi sample size calculator. Data were entered and cleaned using the Statistical Package for social sciences (SPSS). For the categorical analysis, the chi-square test was applied. In the present study, the population's mean age and standard deviation were 32.85 ± 9.57, and 60.2% (n-254) were males. Among the participants, 58.1% (n-245) were aware of the difference between Family Medicine specialists and general practitioners, 62.8% were about chronic disease management included in family medicine clinic and approximately 45.7% (n-193) were aware of psychiatric services included in family medicine clinic. There was significant association was observed between the male gender and excellent satisfaction with family medicine specialist accessibility (P<0.05). Based on the study results, near to two-thirds of the population is aware of family medicine clinics at PHCC and also chronic disease management at family medicine clinics. Close to half of the people are aware of basic psychiatric case management at a family medicine clinic. Need more awareness among the public to utilize quality services from the family physicians working at PHCC and it indirectly decreases the load and waiting time on specialists working at hospitals.
