Design, synthesis anticytokine storm and molecular docking studies of N-benzyl phenyl isoserine derivatives containing 1,2,3-triazoles

Abstract : Cytokine storm is several disorders of immune dysregulation characterized by constitutional symptoms, systemic inflammation, and multiorgan dysfunction that can lead to multiorgan failure if inadequately treated, 1,2,3-Triazoles are one of the most important nitrogen-containing five-membered heterocycles and have a wide range of applications in pharmaceuticals, supramolecular chemistry, organic synthesis, chemical biology and industry. In the current work, the synthesized compounds (1d and 2d) were screened for their anticytokine storm by assay of IL-1b and IL-6 in vivo, confirmations and characterization of the chemical structures related to these compounds were performed using 1H-NMR spectroscopy, FT-IR spectroscopy, and some physicochemical properties such as melting points. Docking study of the final synthesized compounds gave evidence about the affinity of these compounds significantly inhibited IL in comparison to positive groups.

THE THERAPEUTIC EFFECTS OF MAGNESIUM SUPPLEMENTATION ON GLYCEMIC CONTROL AND LIPID PROFILE IN TYPE II DIABETES

Abstract : Individuals with type II diabetes mellitus use dietary supplements on a regular basis. The aim of study was to investigate the therapeutic effect of magnesium supplementation on glycemic control and lipid profile in adult type II diabetes mellitus patients. Seventy type II diabetes patients were allocated into: The intervention group (n=35) was received oral Magnesium oxide 250 mg once daily, and the control group (n=35) was received corn starch as a placebo. The duration of treatment for both groups was two months. After 2 months, the magnesium treated group demonstrated a significant reduction in FSG, HbA1c%, VLDL-C and triglyceride level, while insignificant change in TC, LDL-C and HDL-C, when compared to their pre-supplementation levels and the control group. There is no statistically significant difference between the two studied groups in the mean of age, BMI, systolic and diastolic blood pressure. The intervention group had a significant reduction in FSG (p= 0.034), HbA1c% level was significantly reduced in the magnesium treated group (p = 0.01) while in the control group HbA1c level is increased. Serum triglyceride levels decreased significantly (p < 0.05) in the magnesium supplemented group, but it is increased in the control group (p=0. 54). Oral magnesium supplementation has been shown to be effective in lowering glycaemic control and serum triglyceride level in patients with type 2 diabetes mellitus, and thus may be used as adjuvant therapy for management of patients with type II diabetes mellitus.

Effectiveness of integrated education and relaxation program on migraine related disability; a Randomised controlled trial

Abstract : Migraine is the most disabling primary headache disorder worldwide. The value of integrative strategies in migraine management has been raised due to the recurrent and provoked nature of migraine. To evaluate the effectiveness of implementing therapeutic patient education and relaxation training program versus usual pharmacological treatment alone on the frequency, severity, and duration of migraine attacks as primary outcome and migraine-related disability and quality of life as secondary outcome. A randomized controlled trial was conducted at the specialized headache clinic of a Tertiary Referral Centre. Sixty patients were randomly assigned to intervention or control groups. Participants in the intervention arm received the education and relaxation training program and were instructed to perform daily relaxation exercises in addition to their routine pharmacological treatment meanwhile the control group only received their routine treatments. Follow-up was done after 1, and 3 months using headache diary and migraine specific quality of life questionnaire (MSQ). After implementation of the program there was a significant reduction in migraine attacks severity in the intervention group compared to the control group, also they had significantly fewer migraine headache days/month and duration of migraine attacks compared to patients in control group. Statistically significant improvement of Role-Function Restrictive, Role-Function Preventive and Emotional Function domains of MSQ. An integrated migraine management program has a significant effect on reducing the burden of migraine attacks and improving the daily activities of migraine sufferers.

Application of Stress Management on Quality of Life of Diabetes Mellitus Patients at Diabetes Center, Ternate, Indonesia

Abstract : Diabetes mellitus is a progressive disease that can cause various complications, both acute and chronic if not managed properly. resulting in various chronic complications, such as cerebrovascular disease, coronary heart disease, leg vascular disease, eye, kidney and nerve disorders. People with diabetes mellitus have a 2 times greater risk of developing coronary heart disease and cerebral vascular disease, 5 times more likely to suffer from ulcers/gangrene, 7 times more likely to develop terminal renal failure, and 25 times more likely to be blind due to retinal damage than patients. non-diabetic. The purpose of this study was to determine the effect of the application of stress management on the quality of life of people with Diabetes Mellitus in the Diabetes Center, Ternate City. This research was conducted in the working area of the Diabetes Center, Ternate City. The design of this study was a quasi-experimental study, which was a pre and post-test design without a control group. Participants in this study were selected using a technique using simple random sampling with a total sample of 61 people, where the data were analyzed using the t-test statistical test, Anova and correlation. The results of the study found that there was a significant difference between the quality of life of DM sufferers before and after the application of stress management. The patient's quality of life score also increased after stress management was carried out in the form of deep breath relaxation techniques and five-finger hypnosis (p-Value: 0.0005 α: 0.05), while the characteristics of the respondents included gender, marital status, education, family history, and medication do not affect the quality of life.

Evaluating the clinical significance of Osteoprotegerin Serum Levels as a Predictive Marker in Rheumatoid Arthritis

Abstract : Laboratory tests such as the erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and Immunoglobulin M (IgM) have been used as markers of inflammation and disease activity in rheumatoid arthritis (RA), although there is still no clear consensus on when to use one, the other, or all. We aimed to evaluate the level of OPG in RA patients compared with control in order to estimate the predictive value of OPG. Eighty five patients with active RA, attending rheumatology department at Al-Sadder General Hospital in the province of Maysan-Iraq, diagnosed according American College of Rheumatology (ACR) revised criteria were included. The patients' tender and swollen joint counts were calculated. Laboratory investigations were done including ESR by Westergren method, CRP, IgM and OPG by ELISA method, assessment of disease activity using DAS28 score. A non-significant change (p>0.05) was seen in the level of BMI, significantly increased (p<0.01) in levels of ESR, CRP, IgM, and OPG as compared to the control group. All patients showed disease activity at the time of the study, their DAS28 scores ranged from 5.20 to7.40 (Mean ±SD 6.43±0.80). There was a positive and highly significant (p< 0.01) correlation between DAS28 values and ESR, CRP, IgM, and OPG values (r-value was 0.662, 0.695, 0.689, and 0.636, respectively). The OPG was more specific than Rf (IgM, CRP, and ESR) in the diagnosis of RA. So it could be a useful assay in establishing the diagnosis of RA, especially in ambiguous cases or RF negative patients with RA, and also a better predictor of disease severity.
